I created this account to say thanks to AbbyZeo. I believe your chords are spot on for those parts.
I found out yesterday there is actually a way to do the downward slide on ukulele (like they do with guitar) after Em. The slide chords are D#m, Cm, Am, F#m. It involves starting way up on the fret board (my left Index Finger was on the 9th fret of the A String). 2 out of the 4 chords required leaving the G string out of the equation entirely to correctly produce them. For my uke anyway, this method doesn't seem to generate enough volume to work well. It might be better suited for an electric ukulele (plugged in).
